? Valid state ID card
? Completed new patient packet.
Patient appointment?cancellations should be done at least 24hrs. prior to appointment date.
Otherwise the patient might risk not being accepted into practice.
Referring physician will also be notified for missed appointments.
Please arrive at least?30 minutes prior to appointment time
in order to complete required documentation.
We do not prescribe narcotics.
